All requests for medical records must be in writing with a current (not more than six months old) signed authorization from the patient. You may fax your request to (254)772-0050 or mail it to 342 Richland West Circle, Waco, Texas 76712, Attn: Medical Records.

Medical record requests are processed weekly from our office by a third party medical records processing company. Information contained in our medical files is strictly confidential and will not be released to any person or agency without written consent from the patient.

According to the Texas Medical Association guidelines for release of medical records, fifteen (15) days must be allowed for the processing of medical records. Due to the efficiency of our system records are normally processed within ten (10) working days.

The charge for medical records varies from chart to chart. A prepayment will be requested. If you have any questions regarding the charge you may call (877)746-8678.

TRANSCRIPTION

Ronda Wilkoff is our medical transcriptionist. Her extension is 213. She types all of the doctor’s office visit notes and medical narratives. She will have the dictation ready approximately two days after your visit. Ronda will send the notes directly to your family physician, referring physician or your Worker’s Compensation insurance carrier.
